How do I get my breast pump FREE through my insurance from Northern Pharmacy & Medical Equipment?
Fill out our Breast Pump Prescription Form and have your physician sign and date the bottom portion.
Complete the Northern Pharmacy & Medical Equipment Breast Pump Prescription Form and have your physician sign and date it.
Fax the completed form to 410-254-5134 OR email to breastpumps@northernpharmacy.com. (a photograph of the signed document is acceptable)
Upon receipt, a breast pump team member will contact you. You can email us for confirmation, as well, at breastpumps@northernpharmacy.com.
Northern Pharmacy & Medical Equipment will contact your health insurance company, on your behalf, to confirm coverage and complete your order. We accept most insurances* (private plans AND medical assistance MCO's). Breast pumps are dispensed upon notification of baby’s arrival.
Your equipment will be delivered right to your front door, FREE OF CHARGE*, after order processing, typically within 24-48 hours upon receipt of prescription/notification of baby’s arrival.
The Affordable Care Act (ACA) requires most health insurance plans to cover the cost of your breast pump.
